Annual Reports
If you have been requested to fill out one of our annual reports you can find that form on this page. Please select the form you are responsible for. The forms are fillable pdf documents. You can download and fill out the forms and return them via email. You can also print them, hand-write the answers and send them through the mail. Thank you for taking the time to complete each form. These forms are used for a variety of purposes at the yearly meeting level. We expect each form to be completed to the best of your church’s ability.

*Licensed Minister Minute
NWYM requires a minute from the Clerk of Elders at an Elders meeting from the local church to affirm the approval for a licensed minister.

Credentialing of Ministers
This Credential Handbook is intended for the use of NWYM Local Churches to provide guidelines and objectives for the credentialing of ministerial leadership and those called to ministry roles within the churches of NWYM

First Time Ministry License Application
Elders of local churches may request ministry licenses (certificates of ministry) for members who are not recorded ministers to perform special types of service for the church
